2009 Epilepsy Summer Camp Financial Aid Information

Dear Parent:

The Epilepsy Foundation is pleased to be able to offer financial assistance to make it possible for children whose families have limited financial resources to attend camp.

The following procedure for receiving a campership must be adhered to exactly:

  1. Return the enclosed application to me with the doctor's statement attached . This is a simple statement from the physician saying, Your child's name entered here has been diagnosed with a seizure disorder and would benefit from summer camp." This is NOT the detailed medical form which the camp will require.
  2. Be sure to indicate which camp you have chosen. All scholarships are for one week and one camp only. Please request only the amount you cannot afford to pay,as we will be able to fund very few full camperships. If you do not need funding, go ahead and send your child's application directly to the camp. (Please let me know whether you are choosing Lakewood or Big Heart, and I will send you their packet which you can fill out and return directly to the camp). NOTE: If you are requesting a campership, you should not apply to camp until your application is approved. Make sure you write EFGA at the top of the camp application.
  3. With our "Camp Application for Financial Assistance" you must include the letter stating why you need financial help (i.e. medication expenses, time lost from work, unemployment) We are not asking you to share your confidential financial records.. Your application will not be considered without this letter.
  4. You must agree to Sections A, B, and C and initial each. As stated in B, on partial camperships you must pay your portion to the Epilepsy Foundation of Georgia no later than June 4. Your check to the Epilepsy Foundation of Georgia (EFGA) should have in the lower left hand corner in big letters "CAMP" so that your check is properly credited.
  5. The following are the dates for these camps:
Big Heart (ages 26 and up) June 21 - 26, 2009
  (7 years to 25) June 14 - 19, 2009
Lakewood   June 22-27, 2009


Please consider carefully the amount that you can pay. The more each person pays, the more campers we can fund. Please be prompt in returning your application, as space and funds are very limited .
