1)
If your child is in need of financial assistance, please attach a letter stating the reasons that assistance is needed . The application will not be considered without this letter.
2) You MUST AGREE to the following before your child can be considered for funding.
a) Write a thank you note to the sponsoring group or organization and send it to the Epilepsy Foundation as soon as
your
child returns from camp.
b) The Epilepsy Foundation helps young people to go to camp by providing partial
scholarships, so that the maximum
number of people will have the opportunity to
attend camp. You must agree to pay a pre-determined amount by a
specified deadline, depending on what
your family can afford.
c) Send a Doctor’s Statement attached to this application stating that the doctor
recommends my child go to camp.
